Is your child 18 or older? To maintain access to their health information, please complete the Release of Information form. This form must be completed for your child to share their personal information with a family member, even if your child is still on a parent’s health plan.

5 Emotional Wellbeing Solutions counseling sessions on us
Get up to 5 Emotional Wellbeing Solutions no-cost counseling sessions for you and your eligible family per topic. For help getting connected to a counselor (in-person or virtually), call a Here4TN specialist 24/7 at 855-Here4TN (855-437-3486) or live chat during business hours.
State and Higher Education: Emotional Wellbeing Solutions is available to all benefits-eligible employees and their eligible family members, even if they are not enrolled in medical insurance.
Local Ed and Local Gov: Emotional Wellbeing Solutions is available to employees who are enrolled in medical insurance. Dependents are eligible even if they are not enrolled in medical insurance.
The five Emotional Wellbeing Solutions visits per year, per issue are per individual. Members are ineligible for Emotional Wellbeing Solutions visits while they are currently receiving Behavioral Health services. All members (employees and dependents) enrolled in medical insurance are also eligible for behavioral health benefits.
